Humboldt Independent Practice Association experienced a data breach last year involving unauthorized access to its systems, which resulted in the acquisition of protected health information. The breach was identified on June 28, 2024, when the company discovered a phishing attack posed as a legitimate communication from one of its partners. The unauthorized access spanned from June 26 to July 1, 2024, affecting a single email account. Subsequently, an investigation by external forensic specialists revealed that the compromised data included individuals' first and last names along with other sensitive information like contact details, dates of birth, driver's licenses, medical diagnoses, and health insurance information.
An online data breach notice also indicates that Social Security numbers may have been impacted in the Humboldt Independent Practice Association data breach, as the company is offering credit monitoring services to individuals who had this piece of information exposed.
The company stated that it collects protected health information from providers in the course of administering health plans and healthcare services. Humboldt IPA provides administrative, claims processing and medical management services for Anthem Blue Cross HMO, Blue Lake Rancheria Medical and Dental Health Plans, CalPERS Blue Shield HMO, and IPA Employee Assistance Program.
