The official website for the Bridgeway Center data breach settlement can be found at BridgewayCenterSettlement.com.
The deal, which was granted preliminary approval by the court on September 11, 2024, covers anyone in the United States whose personal information was impacted by the data breach, including those who Bridgeway Center notified about the incident in May 2024.
Only class members who submit a valid claim form online or by mail by February 8, 2025 will be eligible to receive a Bridgeway Center settlement payout.
You may file a claim form online on this page. To submit a claim by mail, you can download the PDF claim form or contact the settlement administrator to request a paper copy.
To file a claim online, you will need the unique ID and PIN printed on the personalized settlement notice you should have received in the mail. The website directs class members to get in touch with the settlement administrator by calling 1-888-453-8305 or emailing info@BridgewayCenterSettlement.com if they did not receive a notice or have lost it.
As part of the deal with Bridgeway Center, eligible class members may submit a claim for up to $1,500 per person for “ordinary” out-of-pocket expenses that were incurred as a result of the data breach and are supported by documentation, the site relays.
Additionally, consumers can submit a claim for reimbursement of documented “extraordinary” losses incurred after the breach that are “fairly traceable” to the incident, for up to $5,000 per person, the website shares.
In lieu of the aforementioned benefits, a class member may instead opt for a $125 cash payment, the site explains.
Per the settlement website, all payments may increase or decrease on a pro rata basis.
The court will determine whether to grant final approval to the terms of the class action settlement at a hearing scheduled for January 24, 2025 in Florida. Cash payouts will be distributed to eligible class members if the deal is ultimately approved and after any appeals are resolved, the site notes.
According to the lawsuit against Bridgeway Center, negligent cybersecurity on the part of the mental health service resulted in unauthorized access to its computer network between February 21 and February 23, 2024. The class action suit claimed the cyberattack may have compromised highly confidential employee and patient data, including at least names, addresses, Social Security numbers, financial information, details about treatments and diagnoses, medical insurance and billing data and other health-related records.
