A data security incident at South West Family Medicine Associates, PA has affected 36,959 individuals, according to the Maine Attorney General's Office. The incident was discovered by the Dallas, Texas healthcare provider around August 7, 2024. Following a forensic investigation and internal document review, the company determined on October 24, 2024 that personal data had been subjected to unauthorized access or acquisition during the data breach.
The data potentially affected includes Social Security numbers, driver's license numbers, dates of birth, addresses, zip codes, medication information, medical history, lab results, diagnosis or condition details, passwords, personal identification numbers, or other access codes used for financial resources. Additionally, passport numbers, mothers' maiden names, unique private keys for electronic records, tax identification numbers or IP PINs, and health insurance policy numbers or subscriber information could also be involved in the Southwest Family Medicine Associates data breach.
