Lawsuit Investigation
Attorneys working with ClassAction.org are looking into whether a class action lawsuit can be filed in light of the Provation data breach.
As part of their investigation, they need to hear from individuals who received a notice stating they were impacted.
Provation Software, Inc., which provides documentation and information management solutions to hospitals and other healthcare providers, is notifying individuals whose personal information may have been exposed to unauthorized access.
In a sample data breach notice provided to the Montana attorney general (which can be viewed below), Provation stated that in October and November 2023, the company detected unusual activity in its systems stemming from two separate unauthorized parties. Following an investigation, it was determined that the unauthorized parties had gained access to Provation's network and acquired data from January 25, 2023 to November 6, 2023, when their access was terminated. Personal information that may have been exposed in the Provation Software data breach includes consumers' names, dates of birth, Social Security numbers, driver's license or state-issued ID numbers, passport and birth certificate numbers, financial account information, credit and debit card numbers, and health insurance information.
According to a separate sample data breach notice provided to the Massachusetts attorney general, Provation is an affiliate of Fortive Corporation, an industrial technology company headquartered in Everett, Washington.
