As part of their investigation, they need to hear from individuals who received a notice stating they were impacted.
Mid-Ohio Psychological Services, Inc., a provider of mental health and substance abuse support, has disclosed a data breach affecting 40,345 individuals.
The breach has been reported to the Texas Attorney General and the U.S. Department of Health and Human Services (HHS). Data breaches specifically involving protected health information must be reported to the HHS Secretary.
According to the Texas Attorney General's website, however, information exposed in the Mid-Ohio Psychological Services data breach is comprised of individuals' names, addresses, Social Security numbers, financial information, and dates of birth.
