Lawsuit Investigation
Attorneys working with ClassAction.org are looking into whether a class action lawsuit can be filed in light of the HCF Management data breach.
As part of their investigation, they need to hear from individuals who received a notice stating they were impacted.
HCF Management, Inc., which manages nursing homes and other care facilities in Ohio and Pennsylvania, has reported a data breach that may have exposed residents' personal and medical information.
While the Erie Times-News reports that all health care establishments managed by HCF appear to be affected by the data breach, notices have been posted on at least 18 of the company's 31 facilities. These senior care facilities are as follows:

- Bradford Manor
- Celina Manor
- Corry Manor
- Edinboro Manor
- Fairview Manor
- Fox Run Manor
- Garbry Ridge
- Hempfield Manor
- Piqua Manor
- Shawnee Manor
- St. Catherine’s Manor of Fostoria
- St. Catherine’s Manor of Washington Court House
- The Manor at Greendale
- The Manor at Perrysburg
- Village at The Greene
- Wapakoneta Manor
- Warren Manor
In addition, HCF Management's EncompassCare therapy and rehabilitation center in Lima, Ohio, has also reported that it experienced a data breach around the same time as the above-listed facilities.
HCF reported that a third party accessed its computer systems on September 17, 2024 and potentially acquired certain files. Upon discovering the breach around October 3, the company took measures to secure its network and engaged a third-party forensic firm to investigate. It was revealed on November 19, 2024 that the compromised data may include sensitive information such as residents' names, addresses, phone numbers, dates of birth, Social Security numbers, medical treatment details, and health insurance information, though the affected information varies by person.
Each impacted facility is sending letters to residents whose information may have been exposed in the data breach. An example notice from the Warren Manor can be seen below.
