The information compromised in the Erie data breach may include names, addresses, phone numbers, email addresses, Social Security numbers, driver’s license/state ID numbers, taxpayer ID numbers, passport numbers, financial account information and payment card information. Other data potentially exposed includes online account credentials, digital signatures, biometric data, dates of birth, treatment or diagnosis information, prescription information, dates of service, patient and encounter ID numbers, provider names, patient account and medical record numbers, Medicare/Medicaid numbers, health insurance information, and treatment cost information. The data affected varies by individual.

Erie Family Health Centers will notify potentially affected individuals for whom it has address information by mail.
