Attorneys working with ClassAction.org are looking into whether a class action lawsuit can be filed in light of the Covenant Care data breach.
As part of their investigation, they need to hear from individuals who received a notice stating they were impacted.
Covenant Care California, LLC, which provides skilled nursing and home healthcare services in California and Nevada, has notified potentially affected individuals about a significant data breach that compromised sensitive personal information. The breach involved unauthorized access to Covenant Care’s network, impacting several files containing data on current and former patients, as well as responsible parties of patients who received services from a facility or agency operated by Covenant Care.
The incident, identified on November 14, 2023, revealed that unauthorized individuals accessed and copied files between November 12 and November 14, 2023. While the exact details of the compromised information are still under review, the affected files may include individuals’ names, dates of birth, medical information, health insurance details, Social Security numbers, financial account numbers, credit/debit card numbers, driver’s license numbers, and state/federal ID numbers.
Sample notification letters sent to those affected by the Covenant Care data breach explain that the company previously provided services through AFFIRMA and currently provides home healthcare services under the following names: Focus Health, Elevate Home Health, Choice Home Health Care, and San Diego Home Health.
